Mesothelioma is a type of lung cancer that is caused by exposure to asbestos.  Exposure to asbestos occurs primarily in occupational settings.  Plumbers and insulation workers were widely exposed for many years.  Each year about 3,000 people will die from mesothelioma as a result of asbestos exposure.  This cancer was very rare a few decades ago; however, it is being seen more frequently as men and women who were born in the 1920's and beyond advance into the older age group.&lt;br /&gt;&lt;br /&gt;There are many types of cancers. This new study, called the Age-Related Eye Disease Study 2 (AREDS2), will build upon results from the earlier Age-Related Eye Disease Study (AREDS). The original study results were released five years ago today. The study found that high-dose antioxidant vitamins and minerals (vitamins C and E, beta-carotene, zinc, and copper), taken by mouth, reduced the risk of progression to advanced AMD by 25 percent, and the risk of moderate vision loss by 19 percent.&lt;br /&gt;&lt;br /&gt;--------------------------------------------------------------------------------&lt;br /&gt;&lt;br /&gt;Prevent AMD&lt;br /&gt;The only treatment proven to slow the vision loss of AMD.&lt;br /&gt;&lt;a href="http://www.cure-macular-degeneration.com/"&gt;http://www.cure-macular-degeneration.com/&lt;/a&gt;&lt;br /&gt;--------------------------------------------------------------------------------&lt;br /&gt;&lt;br /&gt;AREDS2 will refine the findings of the original study by adding lutein and zeaxanthin (plant-derived yellow pigments that accumulate in the macula, the small area responsible for central vision near the center of the retina) and the omega-3 fatty acids DHA and EPA (derived from fish and vegetable oils) to the study formulation. The main study objective is to determine if these nutrients will decrease a person's risk of progression to advanced AMD, which often leads to vision loss. Previous observational studies have suggested these nutrients may protect vision.&lt;br /&gt;&lt;br /&gt;"Vision loss from AMD is an important public health issue. This study may help us find a better way to treat this devastating disease," said Elias A. Zerhouni, M.D., director of the NIH.&lt;br /&gt;&lt;br /&gt;AMD damages the macula. As the disease progresses, it blurs the patient's central vision. AMD can take two forms, wet and dry. Wet AMD is caused by the abnormal growth of blood vessels under the macula. This leads to rapid loss of central vision. Wet AMD is considered to be advanced AMD and is more severe than the dry form. Dry AMD, the more common form, occurs when the light-sensitive cells in the macula slowly break down. Untreated dry AMD can progress into wet AMD.&lt;br /&gt;&lt;br /&gt;Paul A. Sieving, M.D., Ph.D., director of the National Eye Institute (NEI) at NIH, said, "Nearly two million Americans have vision loss from advanced AMD, and another seven million with AMD are at substantial risk for vision loss. In the AREDS study, we found a combination of vitamins and minerals that effectively slowed the progression of AMD for some people. Now, we will conduct this more precisely-targeted study to see if the new combination of nutrients can reduce AMD progression even further. This study may help people at high risk for advanced AMD maintain useful vision for a longer time."&lt;br /&gt;&lt;br /&gt;Emily Y. Chew, M.D., study chair and deputy director of the Division of Epidemiology and Clinical Research at the NEI said, "The AREDS2 study is seeking 4,000 people between 50 and 85 years of age with AMD in both eyes, or advanced AMD in one eye. They must be available for yearly eye examinations for at least five years. Until we get the results from AREDS2, we encourage people with AMD to visit their eye care professional to see if they need to take the AREDS vitamin and mineral formulation. Seddon works at the Massachusetts Eye and Ear Infirmary, which is affiliated with Harvard Medical School in Boston.&lt;br /&gt;&lt;br /&gt;Other experts in Australia found that age-related macular problems are rarer in people whose diets are rich in omega-3 fatty acids, which are found in fish, including salmon and mackerel, as well as flax seeds and walnuts.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Fish Oil and AMD - U.S. Study&lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;/strong&gt;&lt;br /&gt;Seddon's team studied data from 681 individual male twins in their mid-70s who were World War II veterans. The group included 222 men with intermediate- or late-stage AMD and 459 with early or no AMD.&lt;br /&gt;&lt;br /&gt;The men completed questionnaires about their smoking history, alcohol use, physical activity, diets, and use of multivitamins and supplements.&lt;br /&gt;&lt;br /&gt;"Current smokers had a 1.9-fold increased risk of AMD while past smokers had about a 1.7-fold increased risk" of AMD, compared with nonsmokers, the researchers write.&lt;br /&gt;&lt;br /&gt;The study also shows that men with the highest fish consumption (at least two weekly servings) were 45% less likely to have AMD than those with the lowest fish consumption (less than one weekly serving).&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Fish Oil and AMD - Australian Study&lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;/strong&gt;&lt;br /&gt;The Australian researchers included Brian Chua, BSc, MBBS, MPH, of the University of Sydney's ophthalmology department.&lt;br /&gt;&lt;br /&gt;Chua and colleagues studied nearly 2,900 people aged 49 and older (average age: 63-65 years). Participants completed dietary questionnaires; five years later, they got special photographs of their retinas to screen for AMD.&lt;br /&gt;&lt;br /&gt;Participants who reported eating at least one weekly serving of fish were 40% less likely to develop early-stage AMD during the study, compared with those who reported eating fish less than once a month or not at all.&lt;br /&gt;&lt;br /&gt;People who ate fish frequently were also less likely to have late-stage AMD, the study shows. But that pattern was only seen in participants who reported eating fish at least three times weekly.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Fatty Acid Balance&lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;/strong&gt;&lt;br /&gt;When it comes to reducing AMD risk, striking the right fatty acid balance might be important, note Seddon and colleagues.&lt;br /&gt;&lt;br /&gt;In their study, the reduced AMD risk was mainly seen in people who consumed high levels of omega-3 fatty acids and low levels of an omega-6 fatty acid called linoleic acid, which is found in vegetable oils including corn, safflower, and sunflower.&lt;br /&gt;&lt;br /&gt;Seddon and colleagues aren't against omega-6 fatty acids. But they note that a lot of people consume way too much omega-6 fatty acids, compared with omega-3 fatty acids.&lt;br /&gt;"The ideal omega-6/omega-3 ratio is 3:1 to 4:1," Seddon's team writes.&lt;br /&gt;&lt;br /&gt;"However, the average American's diet has an omega-6/omega-3 ratio that ranges from 10:1 to 50:1 … Our results suggest that when our diet is rich in these omega-6 fatty acids (as measured here by linoleic acid), the protective effect of omega-3 fatty acids is dampened."&lt;br /&gt;&lt;br /&gt;Chua's Australian study couldn't confirm those results.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Study's Limits&lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;&lt;/strong&gt;&lt;br /&gt;The studies don't prove that not smoking or eating fish prevented AMD.&lt;br /&gt;Both studies were purely observational; participants weren't asked to quit  smoking or change their diets. The studies also don't specify what type of fish participants ate, or how the fish was prepared.&lt;br /&gt;&lt;br /&gt;Still, the results held after adjusting for other AMD risk factors.&lt;br /&gt;&lt;br /&gt;Inflammation might partly explain the results, the researchers note. It is a disorder resulting in slow and progressive damage to an area of the eye called the macula.&lt;br /&gt;&lt;br /&gt;There is a macula in each eye and the macula is part of the retina. This area is the most sensitive vision processing center in the eye.&lt;br /&gt;&lt;br /&gt;The retina is the portion of the eye that sends light to the brain. The macula allows for very fine and clear vision.&lt;br /&gt;&lt;br /&gt;The disease can occur in one eye or both; however, if it develops in one eye, there is a higher risk that it will develop in the other eye as well.&lt;br /&gt;&lt;br /&gt;Understanding that the disease is one that is progressive is very important. Once you have been diagnosed with the illness, there is only one thing that can be done to prevent the disorder from worsening.&lt;br /&gt;&lt;br /&gt;I'll get back to this in a moment, but first it's important to understand more about AMD.&lt;br /&gt;&lt;br /&gt;There are two types of AMD. There is the wet form and the dry form.&lt;br /&gt;&lt;br /&gt;The wet form consists of only about 5% - 10% of AMD cases and usually presents with some distortion of vision and lines might look wavy instead of straight.&lt;br /&gt;&lt;br /&gt;It is caused by the formation of new blood vessels under the macula that leak; thus the wet form.&lt;br /&gt;&lt;br /&gt;Some patients with wet AMD can lose significant vision in a very short period of time. All of the current medical therapies have been directed at the wet form of AMD.&lt;br /&gt;&lt;br /&gt;What is Dry AMD?&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;The dry form is the most prevalent and is 90% - 95% of the AMD cases and is caused by the break down of the light sensitive cells in the macula. It is more slowly progressive.&lt;br /&gt;&lt;br /&gt;The most common symptom of dry AMD is slightly blurred vision. You may have difficulty recognizing faces. You may need more light for reading and other tasks. Dry AMD generally affects both eyes, but vision can be lost in one eye while the other eye seems unaffected.&lt;br /&gt;&lt;br /&gt;One of the most common early signs of dry AMD is drusen. Drusen are yellow deposits under the retina. They often are found in people over age 60. Your eye care professional can detect drusen during a comprehensive dilated eye exam.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Dry AMD has three stages:&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Early AMD. People with early AMD have either several small drusen or a few medium-sized drusen. At this stage, there are no symptoms and no vision loss.&lt;br /&gt;&lt;br /&gt;Intermediate AMD. People with intermediate AMD have either many medium-sized drusen or one or more large drusen. Some people see a blurred spot in the center of their vision. More light may be needed for reading and other tasks.&lt;br /&gt;&lt;br /&gt;Advanced Dry AMD. In addition to drusen, people with advanced dry AMD have a breakdown of light-sensitive cells and supporting tissue in the central retinal area. This breakdown can cause a blurred spot in the center of your vision. Over time, the blurred spot may get bigger and darker, taking more of your central vision. You may have difficulty reading or recognizing faces until they are very close to you.&lt;br /&gt;&lt;br /&gt;If you have vision loss from dry AMD in one eye only, you may not notice any changes in your overall vision. With the other eye seeing clearly, you still can drive, read, and see fine details. You may notice changes in your vision only if AMD affects both eyes. There is nothing new whatsoever about carpal tunnel syndrome.  Human beings have had carpal tunnel syndrome for as long as there have been carpal tunnels.  The first open carpal tunnel release was described in 1947 and nothing changed very much for 50 years until the advent of the endoscopic procedure in 1990.&lt;br /&gt;Along about the same time, the media publicized the fact that some individuals involved in repetitive type work activities, such as those who work on computers all day, have an increased incidence of carpal tunnel syndrome.  In point of fact, most people who come in the hand surgeon’s office with carpal tunnel syndrome are perplexed as to why they have this disease because they do not engage in classical repetitive type work activities. &lt;br /&gt;&lt;br /&gt;Carpal tunnel syndrome is easily understood if one begins with the anatomy. The carpal tunnel is formed by a semi-circle of carpal bones on three sides. The fourth side that forms the carpal tunnel is the transverse carpal ligament.  The ligament cannot stretch.  Thus the carpal tunnel is a defined space that cannot enlarge.  There is only so much room in that opening. Through that opening passes the median nerve, nine tendons, and spongy tissue around the tendons called tenosynovium.  We start our lives with that extra space.  When we run out of extra space due to the swollen tenosynovium then pressure is placed on the nerve.  When this happens, one begins to develop carpal tunnel symptoms. &lt;br /&gt;&lt;br /&gt;Classic textbook carpal tunnel syndrome symptoms are tingling and numbness in the thumb, index and middle finger (median nerve distribution), aching in the forearm which can radiate to the shoulder and clumsiness or weak grip.  Only about one or two patients out of ten presents with a classic textbook carpal tunnel picture.  Some present with tingling in all fingers while others present with tingling only in the thumb or the middle finger. He was elected to this position by his colleagues. He also serves on many overseeing committees including the Medical Executive Committee, Quality Assurance Committee, Family Medicine Quality Committee, as well as other governing bodies.&lt;br /&gt;&lt;br /&gt;He has had a private medical practice for nearly 30 years. He is a 1975 graduate of the prestigious University of Michigan Medical School. Today he is interviewed by Andrew Cook from California.&lt;br /&gt;&lt;br /&gt;Mr. Cook: Dr. Ringold, do you see a lot of patients suffering from arthritis?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: Yes, Andrew, I see patients suffering with this difficult affliction on a daily basis. It can vary from annoying to completely debilitating depending on the patient.&lt;br /&gt;&lt;br /&gt;Mr. Cook: How do you treat these patients?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: Prior to late in 2004, I always used prescription medication. However, when the popular medication called Vioxx was withdrawn from the market, I decided to take a strong look at my approach to arthritis treatment.&lt;br /&gt;&lt;br /&gt;Mr. Cook: What changes have you made?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: Traditionally for the first 20+ years or so of my practice I would use medications from a class of drugs called NSAIDS. These drugs treated both the inflammatory source of the pain as well as the pain. They were known to be potentially harsh on the stomachs of some people so they were used with caution in people with history of ulcer or stomach irritation.&lt;br /&gt;&lt;br /&gt;Mr. Cook: You said you used them for 20+ years, then what happened?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: A new class of drugs emerged called COX-II drugs. These drugs were presented to us as physicians as drugs that would greatly help arthritic patients without such a high risk of stomach difficulty. Most physicians began prescribing these in high volumes in order to avoid the difficult complication of gastrointestinal bleeding that came along with the NSAIDS.&lt;br /&gt;&lt;br /&gt;Mr. Cook: Did the COX-II drugs work well?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: Yes, these drugs worked very well; however, in 2003 there began to be some talk that Vioxx might have some potentially bad side effects. These side effects had to do with the heart and heart failure.&lt;br /&gt;&lt;br /&gt;Mr. Cook: Is that why Vioxx was recalled?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: Yes, the drug was recalled after it was shown that it increased the risk of heart attack or stroke when compared to other arthritis drugs.&lt;br /&gt;&lt;br /&gt;Mr. Cook: Were there other drugs in this class that you could use?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: Yes, there are other drugs in this class; however, I have become afraid of all of the prescription drugs for arthritis as well as the over the counter anti inflammatory drugs. All of the drugs have been shown to be associated with an increased risk of heart attack or stroke.&lt;br /&gt;&lt;br /&gt;Mr. Cook: What do you prescribe now?&lt;br /&gt;&lt;br /&gt;Dr. Ringold: I try to recommend &lt;a href="http://www.ringoldmd.com"&gt;glucosamine or glucosamine plus chondroitin sulfate&lt;/a&gt;. These are natural supplements that have not been shown to be associated with these horrible side effects. I also recommend over the counter acetaminophen. It usually involves the areas between the toes, and it may extend to the top, side, or even the bottom of the foot.  It rarely involves the ankle or any area above the ankle.  There will be rather intense itching, and the patient suffering from athlete's foot will often awaken during the night scratching the feet or rubbing them on the sheets in an effort to ease the itching.&lt;br /&gt;&lt;br /&gt;The problem is treatable with the application of anti-fungal creams.  Most patients who have difficulty treating their athlete's foot problem simply have not treated long enough.  There is a temptation to discontinue treatment after the initial itching has settled down and the redness and blisters have begun to heal.  Athlete's foot needs to be treated for at least three weeks.  The cream that is being ordered for your condition should have your feet looking and feeling like new by 10 days; however, you must continue to use it three times daily for a full three weeks.&lt;br /&gt;&lt;br /&gt;I then recommend that you use it every time you shower for another three weeks.  In almost all cases this is sufficient to defeat the fungus that causes athlete's foot.  If your problem recurs after this treatment plan, then you must be treated with oral medications which are only available through your physician.&lt;br /&gt;&lt;br /&gt;When patients follow the treatment protocol outlined above, it is rare for me to have to use oral medication.  There are very few skin problems that can be confused with athlete's foot.  However, shingles (herpes zoster) infection can come on very suddenly.  It is a blistering rash, and it may itch as well.  The difference between this and athlete's foot is that shingles is almost always painful.  Another condition that can occur that can mimic athlete's foot is contact dermatitis.  This can occur when the patient is allergic to various dyes either in socks or shoe leather.  This normally would be confined to the shape of the shoe or the sock, and is less likely to occur between the toes.&lt;br /&gt;&lt;br /&gt;The best part about &lt;a href="http://athletes-foot.ringoldmd.com"&gt;athlete's foot treatment&lt;/a&gt; over the Internet is that even if you have one of the other two problems, this treatment cannot hurt you.  It simply will not make things much better.  Therefore, I always ask patients to check with their own physician if they are not better in the proper length of time.
